does this come with an offset drain?

Offset option is not available for this drain. You can look at various PVC fittings to create a custom offset.

Does this drain assembly require a pan liner when used in a concrete recess shower in a monolithic slab? Currently in between plumbing contractor and inspector onnthis issue and creating quite a few issues from a production stand point.

This shower drain is designed for use with pan liner as shown in the installation manual (see "Literature" tab).
